Solution
To create a mobile survey in Ezitracker (EziClassic), you can either watch the instructional video or follow the step-by-step guide below.

Step 1: Create the Survey
Go to the Maintenance tab and click Survey Maintenance.
Select Mobile Surveys.
Click the Add button.
Fill in the following details:
Survey Name
Date Range (when the survey should be available)
Tick the Active box
Tick Mandatory if the survey must be completed
Click Update to save.
Step 2: Add Survey Questions
Click Maintain Items.
Click the Add button.
Enter your question.
Select the days the question should be available.
Tick the Active box.
Choose a Response Setting (see below for options).
Click Update.
Repeat this process to add more questions. You can use the arrows under Order to rearrange the question sequence.
Response Settings Explained
Optional: Allows the employee to skip the question.
Min and Max Score: Sets a numeric range (e.g., 1β10).
Yes/No: Simple binary response.
Free Text: Open-ended response (limited to 255 characters).
RAG: Red, Amber, Green options for nuanced responses.
Photo: Allows photo upload (ideal for documenting hazards or injuries).
Step 3: Use the Cascading Feature
This feature lets you control the flow of the survey based on responses.
Example:
Question: Was home care provided?
If No, skip the next question.
If Yes, proceed to Please state what Home care jobs were completed.
How to set it up:
Click Edit on the question.
A Paths section will appear.
Under each possible answer, choose the next question or select End Survey.
Click Update to save.
Repeat this process for any question where conditional logic is needed.
Step 4: Assign Sites/Clients
To ensure the survey appears only when employees log out of specific sites/clients:
Click Maintain Sites.
View sites/clients under the current manager or select All Managers.
Tick the boxes for the sites/clients you want to include.
Click Add Sites.
Confirm the selected sites/clients appear under Site Name.
Click Back to finish.
Once all steps are completed and saved, your mobile survey will be active and ready for use by employees at the designated sites.
